Ra-Yalovi was the son of an Efrosian diplomatic attaché assigned to Earth. He didn’t take to well to the trappings of the child of a diplomat and got into no end of trouble. His mother, a doctor, tried to tame his wild side but focusing is energy toward healing people instead of hurting them. The challenge of healing people helped at the time, but wasn’t enough. Yalovi was still bitter about being taken from his home world. Growing up bully and being bullied he wasn’t a stranger to violence. He initially joined Starfleet as a means to work out his aggression, and found himself with a marine special tactics squad, where his prior training as an EMT in his late youth was put to good effect. He was the team medic. He learned early in his Starfleet career that God truly loves a medic. After a particularly grueling and violent mission with the marine unit, he was given a Special Commendation, and promoted. However, this promotion took him out of the field and placed into a permanent medical posting as a ships deputy medical officer. During a starship battle, his friend, the chief medical officer of his ship, was killed. This lead to his promotion to acting chief medical officer for a time. His actions once again landed him a promotion, and he was once again reassigned. This time to the USS James T. Kirk. Ra-Yalovi is no stranger to violence, and isn’t surprised by much anymore. While he is the chief medical officer, this doesn’t stop him from assigning himself to most of the away missions. His time as a marine has made him a rough and tumble type of doctor.

How long was he a marine (corpsman?) being getting the battlefield commission?

I am guessing 10 years, then 10 years as a full Doc. Military Doctors general start at O-3.

That would be true if we weren't talking about Starfleet where an ensign can remain an ensign for 7 to 10 years without being thrown out of the service (coughharrykiminvoyagercough coughensignmayweatherandsatoinenterprisecough).
